Used to gather faculty and staff input on the candidates based on the candidate's presentation and associated Q&A session. Forms are created using LibWizard Surveys. Forms can also be created using Qualtrix. The link for the online form is emailed the morning of the candidate presentation and again, at the end of all interviews. Print forms are available below as templates or by request.
The hiring rules as of 2017/2018 state that you only have to do reference checks on your top candidate (unless you want to do more). We do, however, have to write/type up summaries of the answers to the questions and submit a form for each reference check made.
